Enrollment Resources
Find helpful information and resources to guide you through the enrollment process and prepare for services.
Our Client Referral form allows parents, guardians, professionals, or other referral sources to submit information about an individual who may benefit from our services so our team can review their needs and begin the intake process. A referral does not necessarily mean the person is automatically enrolled or accepted into services; It's the first step in the process of determining eligibility, needs, and appropriate services.
Help us enhance our services by sharing your feedback. Click the link above to complete our brief Client Satisfaction Survey and let us know how we're doing. Your insights are invaluable in helping us continue to provide exceptional care and support for our community.
Start your journey with us by completing our Intake Packet, designed to gather essential information to tailor our services to your child's needs. This packet is your first step towards personalized care at our facility.
Explore our Client Handbook to learn more about our agency, including important policies and procedures that guide our services. This handbook is a valuable resource for understanding how we operate and what you can expect during your time with us.
